What are the responsibilities of the various levels of government?

Municipal - Road maintenance, garbage collection, local bylaws, animal control, police, ambulance and fire services, libraries, parks and recreation, municipal property taxes.
 
Provincial - 
Birth/Marriage/Death certificates, Drivers license, Health cards, Ontario Works (social assistance), Workplace Safety and Insurance Board (WSIB), Ontario Disability Support Program (ODSP), Employment Standards (lost wages, severance), Student Loans (provincial) GO Transit, Family Responsibility Office (FRO).

How do I get a health card?

(Renewal of health cards, or to replace a lost or stolen card)

To apply for a card, you must:

  1. go in person to your nearest Ministry of Health and Long-Term Care Office or book an appointment (1-888-376-5197) at an Outreach site (Service Ontario Office [Land Registry Office] at 41 Broadway, Orangeville Unit 7), and
  2. fill in the Registration for Ontario Health Coverage form, and
  3. provide the necessary documents (proof of citizenship, residency (address) and identity (photo ID)

For Dufferin/Caledon residents the nearest OHIP office is located at:

Does the Health Card expire?

Yes. Ontario photo health cards have an expiry date that is linked to your date of birth. The red and white cards do not have an expiry date but the ministry is gradually contacting Ontario residents to switch them over to a photo health card.

If you are older than 15½ years, you will need to visit an OHIP Office or book an appointment at an Outreach site (Unit 7, 41 Broadway, Orangeville) to renew your health card and have a photo taken.

Please note: Children under 15½ years of age will receive a renewal form that can be mailed directly into the Ministry of Health and Long-Term Care. No photo is needed for children under 15½ years.

How do I book an appointment at the satellite office in Orangeville?

How do I obtain a birth, marriage or death certificate?

Applications for birth, marriage or death certificates are available at my constituency offices and online at: www.orgforms.gov.on.ca.

Online applications have the fastest turn around time.

Birth of a Baby - The process for a new birth certificate for a child born in Ontario is a one step process for birth registration, obtaining a birth certificate and Social insurance Number. Visit the Newborn Registration Website at: www.orgforms.gov.on.ca/IBR/start.do

Help, I need a birth certificate – fast!

Sylvia Jones’ constituency office can help in expediting a replacement or new birth certificate (if the birth registration is complete) that is needed for urgent reasons such as travel or employment. The Registrar General’s office requires proof of urgency (plane, hotel or event reservations). Delayed birth registrations cannot be expedited for any reason.

In order to contact FRO you will need your case number and a PIN number. The PIN number is a secure number, known only to you, and must not be shared. If you don’t have a PIN number contact the FRO office.

I am having problems contacting FRO, can you help?

Yes. Contact Sylvia Jones’ constituency office to discuss your issue and in some cases, with consent to release information, we can make enquiries on your behalf. Because of privacy some information may not be made available. The MPP’s office in no way makes decisions for FRO. We can help by facilitating and ensuring communications between FRO and the client.
